Short-Term Success Story

Juan A. was admitted to Woodridge Nursing and Rehab following a hospitalization secondary to pneumonia, which he was not able to make spontaneous recovery. Prior to his hospitalization, he was able to perform functional mobility, gait with modified independence with AD. He was evaluated by our physical, occupational and speech therapists.

They each developed individualized care plans to target his limitations. Which focused on improving his strength, functional, mobility, transfers to increase his safety awareness with gait to increase his quality of life. He was very determined in improving his strength to get better as soon as possible.

Juan A, along with the support of a great nursing and therapy staff has improved in his endurance and strength while performing functional mobility and gait with increased awareness of environmental hazards. Juan discharged home with great family support making significant gains towards all set goals.
